someone you know has gone missing please inform your local police
department immediately. After you have done that please send me as
much information as possible about the missing person and the events up
to his/her disappearance. Please include the following:

- Date of Birth (DOB)
- Height
- Weight
- Nationality
- Hair Color
- Eye Color
- Tattoos
- Scars
- Birthmarks
- Date Last Seen
- Date Reported
Missing
- Police Case Number
- Police Department
Responsible for Case
- Police Phone Number
- Any other info
regarding the missing person
- Pictures (send me as
many as you like)
- City, State or Province, Country of
disappearance
